Pet Reservations
LOT only take a limited number of pets in the cabin, so make reservations for your pet as soon as possible by calling their Contact Center at (+48) 22-577-77-55. If the limit is reached, they can refuse your pet travel.
Pets in the Cabin
LOT welcomes small cats and dogs into the cabin on select flights, as long as the combined weight of the pet and carrier does not exceed 17 lbs (8 kg). During the flight, your pet must remain inside their carrier underneath the seat in front of you. The fees for this service vary depending upon your destination: domestic flights are 12 EUR; flights to Europe and the Middle East are 50 EUR; and flights to USA, Canada, China, and Japan are 70 EUR.
Pets in Checked Baggage
Pets that exceed the weight limit for transport in the cabin can be transported as checked baggage in the hold instead. Your pet must be able to fit comfortably in a carrier. When making your reservation, inform your LOT booking agent of the size of your pet's carrier to see if they can accommodate them on your flight. The fees for this service vary by destination: domestic flights incur a charge of 280 PLN; flights to Europe and the Middle East incur a charge of 150 EUR; and flights to USA, Canada, China, and Japan incur a charge of 290 EUR.
Pets in Cargo
Pets who do not meet the requirements to fly in the hold can be transported as cargo instead. Fees for this service depend on the size of the pet's carrier, the weight of the animal, and the route. Carrier Guidelines
For pets traveling in the cabin, carriers must not exceed 21" x 15" x 8" (55 cm x 40 cm x 20 cm) or 17" x 12" x 8" (45 cm x 30 x 20 cm). They should be large enough to allow the animal to move freely, stand, turn around, and lie down comfortably. LOT requires carriers to be clean, tight, securely closed, and the bottom to be lined with absorbent material from the inside. For pets in the hold, carriers must not exceed 49" x 38" x 18" (125 cm x 96 cm x 64 cm). They should be labelled with a relevant sticker, fitted with a water and a food container inside, and ventilated on at least 3 sides. Carriers in the hold should be made of fiberglass, metal, rigid plastic, or solid wood.
Other Restrictions
If LOT reaches capacity for pets in the cabin and as cargo, they reserve the right to refuse carriage of the pet, which also includes the carriage of guide dogs and emotional support animals.

I flew with my dog back in 2018 and had a great experience. He was too large for the cabin, so he had to be checked. I had to call to book his spot (I think you can now do this online). The check-in process was simple and they helped with any questions! We flew from Warsaw to Chicago and had no issues, they even gave him fresh water at some point! I picked him up in Ohare and he was taken off the plane much quicker than the regular luggage which was great!

The LOT airline seems to have simply and easy to follow policy. My Spaniel puppy was tad over 8kg including his travel bag but they were OK. The space under the Dreamlike seat is sufficient to fit a soft bag that can be squeezed and with extra expandable pocket. The airline does not have a pet relief area at the Warsaw Airport but the dog can walk outside and once past the security he can just walk the airport. Not sure if anyone who will try to travel with ESA on board that is over 8kg will be allowed on board. No info on LOT website if ESA over 8 kg will be allowed.
